Letter - Guidance on Reporting Defective Medicines

If a medicine is suspected to be defective in any way, the guidance in the DHSSPS letter should be followed and an online form completed to report the suspected defect directly to MHRA.  The online form is available at either http://www.dhsspsni.gov.uk/index/pas/defective-medicines.htm  OR
